Are you one of many SNF providers who list reducing their rehospitalizations as one of their top organizational priorities? Section 6102c of the Affordable Care Act provides an excellent framework to achieve this goal. QAPI stands for Quality Assurance and Performance Improvement. One year after a final rule for QAPI was issued , all nursing homes were required to implement an acceptable QAPI plan. Why then not use the very best of QAPI to reduce rehospitalizations? In this session, attendees will learn how to apply specific elements of QAPI and create a structure to implement a system-wide rehospitalization reduction plan.

Outline of the new federal regulation that if being implemented in July 2013 requires direct deposit for all resident checks. Various options on compliance for the new regulation as well as simplifying the complicated details of rep payee, the Direct Express Mastercard and cash flow will be discussed. This session will discuss how to create successful interventions that work for the resident as well as those providing the care. This session will also focus on the impact of dementia and mental illness on day-to-day function, mood, behavior and socialization for residents. This session offers a framework for the development and implementation of practical, non-pharmacological interventions for addressing challenging mood and behavior issues. The session will include case studies built from real life examples and participants will be able to problem solve different scenarios together. Participants will also be encouraged to share real life experiences and brainstorm with colleagues on successful interventions for actual situations in their communities.

Participants will learn tools for finding the right AIT to fit within company/facility culture and how to develop an AIT curriculum & training program which takes into account the experience, strengths and weaknesses of the AIT. Participants will walk out of this course as certified Preceptors and prepared with the tools necessary to create a comprehensive program that cultivates successful AITs.

There are significant changes impacting group health and welfare plans since the passage of the Affordable Care Act. The Coaching Approach to Supervision is a relational approach to managing and supporting direct care workers that helps them to develop problem solving skills such as the ability to think critically, prioritize and communicate effectively. This interactive training will provide supervisors of direct care workers the opportunity to learn the coaching approach to supervision and to begin practicing four important skills, including: active listening, self-management, self-awareness and presenting the problem.
