The 2008 OHCA Annual Convention Awards is a chance to say
“Thank You” to those who go the extra mile for the well-being of
residents and co-workers and show our appreciation for everything
they do. There are seven (7)
OHCA award categories
this year and two (2) awards to be given out by the
Oregon Health
Care Foundation (OHCF).
Last year’s award winners set a high
standard and we look forward to continuing the tradition.
The winners will be honored at the
OHCA Annual Convention & Trade Show Awards Ceremony September 9th,
2008 at the Red Lion on the River in Portland, Oregon. Nominators
and Award Winners will be contacted prior to the Awards Ceremony to
ensure the Award Winner is present at the ceremony.

OHCA Award
Categories & Definitions:
Administrator of the
Year Award
Eligible Nominees: All
current Administrators working in an OHCA member
facility/agency.
Criteria: This award
recognizes the achievements and commitment of a facility
Administrator whose contributions and leadership have improved
the overall facility. The nominee’s length of commitment to
his/her facility, improvements to the facility, improvement in
resident quality of life and positive impact on employee morale
will be closely examined.

Adult Volunteer of
the Year Award
Eligible Nominees: All volunteers, ages 19 or older,
serving an OHCA facility, agency or corporate office member.
Criteria: This award
recognizes the contribution an individual adult volunteer or
group of adult volunteers who have made contributions to the
quality of life for resident, family members, or to the working
environment for staff.

Enrichment of Life Award – Employee/Employee Group
Eligible Nominees: Individual facility staff or group of
individuals working for an OHCA member facility/agency or
corporate office. Criteria: This award recognizes an
individual employee or group of employees for contributions that
have positively influenced the quality of life for residents or
the quality of the working environment for coworkers. The
nominee’s commitment to his/ her employer is closely examined.
If a group is nominated, the description must include the
group’s purpose and general focus and the impact of the group’s
efforts. (Groups may be an employee council, support group,
special interest or other appropriate example).

Enrichment of Life Award –Resident/Resident Group
Eligible Nominees:
Individual resident or group of residents living in an OHCA
member facility/agency.
Criteria: This award
recognizes an individual resident or group of residents/ family
members for contributions that have positively influenced the
quality of life for other residents or the quality of working
environment for facility staff. The nominee’s commitment to the
community is closely examined. If a group is nominated, the
description must include the group’s purpose and general focus
and the impact of the group’s efforts. (Groups may be a resident
council, family council, support group, special interest or
other appropriate example).

Innovative Program
in Customer Service Award
Eligible Nominees: All OHCA
facility, agency, corporate office and business partner members
Criteria: This award
recognizes the efforts of a member to satisfy the needs of
customers through an innovative program or service approach. The
award recognizes the development of a creative or unique program
or service that has enhanced community relations and/or the
quality of operations for a member, its staff, or its residents.

Leadership Award
Eligible Nominees: All
employees in a leadership or supervisory position working for an
OHCA facility/agency or corporate office member.
Criteria: This award recognizes the achievements of an
employee in an administrative, middle management, or supervisory
position. Nominee’s leadership skills must have a positive
impact on employee morale and resident quality of life. The
nominee’s commitment to his/her employees, coworkers and
residents is closely examined.

Teen Volunteer of the
Year Award
Eligible Nominees: All volunteers ages 12 – 18 years
serving an OHCA facility, agency or corporate office member.
Criteria: This award
recognizes an individual volunteer or a group of volunteers who
have made contributions to the quality of life for residents or
family members, or to the working environment for staff.

OHCF Award Categories & Criteria
OHCF Senior Wish
Champion Award
Eligible Nominees: Any person or group working in a long
term care facility.
Criteria: This award recognizes an individual or group
working in a long term care facility whose extraordinary
contribution to fulfilling a Senior Wish (or wishes) facilitated
a successful event and enriched the lives of all residents
living in the facility by uniting the community.
The nominee’s creativity and resourcefulness in wish fulfillment
will be closely examined. Nominees who have demonstrated
broad-based support through individual, resident, family, and
staff participation, outside contribution of resources, and
evidence of building community partnerships will be highly
considered.
Five hundred dollars, to be used to fund future senior wishes,
will be awarded to the long term care facility whose nominee is
the recipient of the Senior Wish Champion Award.

OHCF Community Champion
Award
Eligible Nominees: All individuals, businesses, groups or
community organizations (outside of long term care facilities).
Criteria: This award recognizes contributions of time,
energy, expertise, and/or resources that have enriched the lives
of senior residents living in a long term care facility. The
nominee’s efforts may take the form of educational activities,
cooperative ventures, creative collaborations, volunteer
activities, aiding in a Senior Wish, or other examples of
partnerships that improve quality of life in long term care
settings.
