CCC’s Hooper Detox Center is hiring two Patient Care Coordinators.

Schedule: Monday-Friday 8:00am-4:30pm

The HDSC Care Coordinator works within a multi-disciplinary team providing integrated services to patients seen at Hooper. This individual supports Hooper Inpatient and Outpatient services and programming, including ongoing Medication-Supported Recovery (MSR) services offered through the Hooper Bridge Clinic. The Care Coordinator is responsible for providing administrative support to the treatment team, ensuring care that is accountable, comprehensive, proactive, well-coordinated and patient-centered. A key element of this role is the assessment of patient care-coordination needs and serving to identify and solve for solutions regarding barriers to clinical engagement within the continuum of care.

Essential Duties and Responsibilities:

Successfully coordinate all aspects of participant intake from study initiation until study completion
Perform all responsibilities related to study intake, including but not limited to:
Explaining the study to patients and answering patient questions related to the study
Conducting informed consent for study participation
Collect baseline data using the BEES Baseline Information Form, including entering all information into a web-based system
Manage the BEES random assignment portal operation, including communication with the research partners
Conduct random assignment and inform study participants of the results
Maintain thorough knowledge of study specific inclusion/exclusion criteria
Conduct work with integrity and responsibility
Implement procedures to support the study and its analyses
Maintain research integrity
Support for study monitoring visits
Keep management informed of study status and any study-related issues
Provide progress updates and reports
Cooperate with other study activities being conducted by the Study Team
Timely reporting of study events
Other duties as assigned
Requirements:

Must have a Bachelor’s degree. AA degree and 2 years of experience may substitute for the Bachelor’s degree.
CADC I preferred.
At least one year experience working with homeless, low-income, or other vulnerable populations in a supportive role.
Has the competencies necessary to:
Communicate effectively orally and in writing;
Understand healthcare related terminology;
Provide outreach, engagement and supportive services
Past experience with interviewing, data entry, and project coordination and/or managing research studies preferred
If a recovering chemically dependent person must be in a program of recovery and able to provide documentation of sobriety a minimum of two years.
Exemplary skills in Microsoft Office suite, including Excel, preferred.
Ability to independently track information pursuant to individual clients and their navigation through the continuum of care.
Must have current CPR certification or must obtain before hire
Must pass a pre-employment drug screen, TB test, and background check.
Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects.
Must adhere to agency’s non-discrimination policies.
Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, political affiliations, cultural backgrounds, lifestyles and sexual orientations, and treat all individuals with respect and dignity.
Sufficient manual dexterity and physical ability to perform duties.
