CCC’s Old Town Clinic is seeking a RN Care Team Manager!

Old Town Clinic provides compassionate health care to people who might not otherwise get care, treating them with dignity and respect no matter how complex their care. Located in the heart of the Burnside neighborhood, patients can often get all the care they need in a single location.

The RN Care Team Manager supervises support staff and helps facilitate clinical services within their own care team and provides nursing assessment and evaluation on other primary care teams at Old Town Clinic. The RN CTM is responsible for delegation and supervision of the work of the primary care home, ensuring care that is accountable, comprehensive, proactive, well-coordinated and patient-centered, addressing medical, mental health, addiction, and behavior needs.

Duties will include:

Clinical Care:

Responsible for nursing assessments, evaluation and documentation
Oversee Medical Assistants (MA) and Health Assistants (HA)
Responds to urgent medical, psychiatric and behavioral patient situations using the nursing process as well as trauma informed care and harm reduction principles collaborating with the provider as appropriate.
Delegates, provides direction and/or administers immunizations, and medications as ordered—particularly high risk injectable medications.
Supervises compliance scope of practice.
Coordinates with pharmacy and patients for medication management and education.
Coordinates clinical care and referrals within CCC programs and with community partners.
Team Leadership:

Work as a member of the multi-disciplinary team to deliver comprehensive, client centered and trauma informed integrated services.
Actively participate in the integration of services and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
Proactively identify opportunities for improvement within workflows and logistics. Participate in review of Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
Guides care team in setting and achieving performance goals.
Hires, supervises and coaches MA and HA support staff, supporting their performance and development goals.
Supports clinic wide staffing needs.
Quality and Compliance:

Directs innovation in care team and clinic practice through continued quality improvement initiatives.
Adheres to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.
Requirements:

Current Oregon licensure as a Registered Nurse (RN)
Minimum one (1) year of supervisory experience required. Two (2) or more years of experience preferred.
Minimum two years of nursing experience required.
Minimum one (1) year experience in community health or primary care.
Must have current CPR certification or obtain as a condition of employment
Must pass a pre-employment drug screen, TB test, and background check.
